YOU ONLY NEED TO FILL IN THE BLANKS THAT HAVE A STAR (*) NEXT TO THEM.
The required physical is available on this website in both English and Spanish. It needs to be printed and brought to your doctor. When complete, it needs to be uploaded back to the website. Follow the directions on the website.
The MSHSL eligibility packet is available to fill out online on the last page of the registration process. Follow the directions on the website.
The activity fee still needs to be paid in person at the front desk of Cristo Rey; fees are $50 for your first sport, $25 for your second sport, and $0 for your third sport. You should receive a receipt of payment. Your student needs to bring that receipt to the first practice of the season.
